Building Use Form

Individuals and groups wishing to use BEVC-UMC facilities must complete a Request for Use of Church Facilities. This form and full payment MUST be presented to Karen Butler, Facilities Coordinator. (Payment must be made at the time of the request in order to confirm the reservation.) THIS MUST BE HANDLED THROUGH KAREN BUTLER, Facilities Coordinator.
